Social Worker – Adult Long Term Team

Adult Long Term Team | Hybrid Working | 3 Days On-Site in Barry / 2 Days From Home

We are currently recruiting for an experienced Social Worker to join an Adult Long Term Team, supporting adults with care and support needs to achieve their personal outcomes and live fulfilling lives in a way that matters to them.

The role will involve case management of a caseload of approximately 18 adults, including individuals requiring support through to end of life. You will work closely with citizens, carers and partner agencies to assess needs, coordinate appropriate support and ensure care and support plans remain responsive to changing circumstances.

The Role

As a Social Worker within the Adult Long Term Team, you will:

  • Undertake assessments of adults and carers in accordance with relevant statutory duties and legislation.
  • Support adults with care and support needs to identify and achieve their personal outcomes.
  • Provide appropriate information, advice and assistance to enable individuals to make informed choices about their wellbeing and support.
  • Develop, coordinate and review care and support plans through co-production with citizens and carers.
  • Plan and commission appropriate services and support to help overcome barriers and achieve identified outcomes.
  • Manage a caseload of approximately 18 adults, prioritising workload according to need and risk.
  • Provide ongoing case management and review support as circumstances change, including working with individuals approaching end of life.
  • Work in partnership with adults, carers and adults at risk to promote independence, wellbeing, choice and control.
  • Liaise and work collaboratively with internal teams and external partners, including statutory and voluntary organisations.
  • Negotiate and coordinate applications for appropriate resources and services on behalf of citizens and carers.
  • Maintain accurate and up-to-date electronic case records, ensuring all relevant documentation is completed to a high standard.
  • Contribute to performance management and provide accurate statistical information as required.
  • Identify and respond appropriately to safeguarding concerns and risk.

Working Arrangements

This is a hybrid role, with an expectation of working:

  • 3 days per week on-site
  • 2 days per week from home

What we're looking for

You will need to have:

  • A recognised Social Work qualification.
  • Current registration with Social Care Wales or Social Work England.
  • Experience working within Adult Social Care.
  • Experience of statutory assessment and case management.
  • The ability to manage a varied caseload and prioritise competing demands.
  • Strong communication, assessment and report-writing skills.
  • Experience working collaboratively with adults, carers and multi-disciplinary partners.
  • A good understanding of safeguarding, care planning, wellbeing and strengths-based practice.
  • The ability to work independently while contributing effectively as part of a wider team.
  • A commitment to co-production, person-centred practice and promoting choice and independence.
