Neighbourhood Community Network Team - Adults Social Worker - Full Time

We’re working with Newport City Council to recruit a Social Worker into their Neighbourhood Network Team, a service focused on early intervention, prevention and keeping people connected within their local communities.

This is a great opportunity to step into a team that genuinely values strengths-based practice and relationship-building over box-ticking.

This role is predominantly working from home. You will need to hold a driving licence and have access to a car as you will be visiting clients across Newport.

What you’ll be doing:

• Holding a community-based caseload, supporting adults within your local neighbourhood
• Completing strengths-based assessments, care planning and reviews
• Focusing on early intervention and prevention to reduce longer-term risk
• Supporting individuals to remain independent and connected to their communities
• Working closely with partner agencies, families and local services
• Playing a key role in safeguarding and risk management where required

What we’re looking for:

• Qualified Social Worker (Social Care Wales registered)
• Experience within Adult Services
• Confident with assessments, care planning and safeguarding
• Someone who values relationship-led, community-focused work
• DBS on the update service or happy to apply for one
• 3 years of referencing available
• Car driver with access to a car
