Job Description

Social Worker / Senior Practitioner – Young Adults Transitions Service

Bristol City Council | 5-month contract

Hoop Social Work is recruiting an experienced Social Worker / Senior Practitioner to join Bristol City Council’s Young Adults Transitions Service, supporting young people with additional needs as they prepare for adulthood and work towards greater independence.

The citywide service works with young people aged 14–25, helping them maximise their independence through assessments, occupational therapy support, care planning and access to appropriate services and resources.

This is an excellent opportunity for an experienced practitioner who enjoys working with young people and their families, supporting colleagues and contributing to the development of a specialist transitions service.

The role

As a Senior Practitioner, you will play an important role within the service’s leadership team, supporting Team Managers to develop the transitions service and providing day-to-day consultation, guidance and direction to practitioners.

You will help ensure young people receive appropriate, person-centred support as they move into adulthood, promoting independence and making the best use of commissioned services, charitable organisations, technology and equipment.

You may hold a small caseload of complex and challenging cases, leading by example and modelling best practice. The role also includes supervising staff across the multidisciplinary team, including Social Workers, Occupational Therapists, Social Care Practitioners and Occupational Therapy Aides.

You will need a calm, solution-focused approach, with the ability to manage competing priorities and provide confident professional guidance.

Key responsibilities

  • Complete Care Act assessments for young people with additional needs transitioning into adulthood.

  • Support young people to maximise their independence through appropriate assessments, care planning and occupational therapy support.

  • Develop support plans and contribute to the commissioning of services to meet assessed needs.

  • Identify alternative sources of support, including charitable organisations, technology and equipment to promote independent living.

  • Provide day-to-day consultation, professional guidance and supervision to staff across the multidisciplinary team.

  • Support Team Managers with the ongoing development and improvement of the transitions service.

  • Manage a small caseload of complex cases where required, modelling high standards of practice.

  • Apply relevant legislation and procedures, including the Care Act 2014 and Mental Capacity Act.

  • Contribute to adult safeguarding practice and undertake visits across the city and, where required, out of county.

Working arrangements

  • Contract: 5 months

  • Location: Bristol City Hall

  • Office attendance: Minimum of 2 days per week, likely to be 3 days due to duty manager responsibilities.

  • Home working: Approximately 2 days per week.

  • Service area: Citywide, with some out-of-county visits.

What we’re looking for

  • A recognised Social Work qualification and current registration with Social Work England.

  • At least six months’ experience working as a registered Social Worker in the UK.

  • Experience working with adults or young people with additional needs and supporting transitions into adulthood.

  • Strong knowledge of the Care Act 2014, Mental Capacity Act and other relevant legislation.

  • Previous experience of adult safeguarding.

  • Experience providing supervision, guidance or professional support to colleagues.

  • Excellent assessment, care-planning and risk-management skills.

  • The ability to manage complex cases, balance competing priorities and work effectively within a multidisciplinary team.

  • A commitment to promoting independence, choice and positive outcomes for young people.

Essential requirements

  • A relevant Social Work qualification.

  • Current registration with Social Work England.

  • A minimum of six months’ experience working as a registered Social Worker in the UK.

  • An Enhanced DBS registered on the Update Service, or willingness to apply for one.

  • Three years’ referencing history.

  • A full driving licence and access to a car.

Please note: Sponsorship is not available for this role. Applicants must already have the right to work in the UK.

Why register with Hoop Social Work?

  • Over 20 years’ experience in the social work recruitment market.

  • A specialist agency based in Cardiff, recruiting across South West England and beyond.

  • Access to exclusive roles and early opportunities.

  • PAYE or Umbrella payroll options to suit your circumstances.

  • Twice-weekly payroll.

  • A dedicated consultant as your main point of contact, with wider team support when needed.

  • Earn £200 for every successful friend or colleague you refer.

  • Wellbeing sessions and thoughtful extras, including birthday gifts.

  • Dedicated DBS and compliance support to make the registration process as straightforward as possible.
