Adult Social Worker – Ceredigion Council

Location: Ceredigion – Penmorfa, Aberaeron
Working pattern: Hybrid
Hours: Ideally 37 hours per week
Rate: £30.50 PAYE / £39.35 Umbrella per hour
Contract: Long-term, initially until April 2027

Ceredigion Council is looking for an experienced Adult Social Worker to join their friendly and supportive Triage & Assessment Team.

You’ll be working with adults across Ceredigion, undertaking face-to-face visits, assessments and case management responsibilities, alongside the usual duties associated with an Adult Social Work role.

The team is well established and supportive, with this position providing some additional capacity due to an increase in demand.

The role will involve:

• Undertaking face-to-face visits with adults across Ceredigion
• Completing assessments and identifying individuals’ care and support needs
• Managing a caseload and taking responsibility for case management
• Developing and reviewing Care and Support Plans
• Working alongside families, carers and partner agencies
• Promoting strengths-based, person-centred approaches
• Responding to referrals and supporting the triage and assessment process

What we’re looking for:

We’re looking for an experienced Adult Social Worker who is confident completing assessments, managing cases and undertaking face-to-face visits.

You’ll need:

• A Social Work qualification
• Current Social Care Wales registration
• Previous Adult Social Work experience
• Confidence undertaking face-to-face assessments and visits
• Good case management and assessment skills
• The ability to work independently while being part of a supportive team
• A driving licence and access to a vehicle for work-related travel

£30.50 PAYE / £39.35 Umbrella | Hybrid working | 37 hours | Until April 2027
