£38.43 per hour Umbrella | £30.50 per hour PAYE
3-month contract | Full-time | Hybrid working

Hoop Social Work are recruiting an experienced Social Worker to join a Community Mental Health Team (CMHT) within Swansea Council's Adult Services for an initial 3-month contract.

This is an excellent opportunity for a Social Worker looking for a role that offers a good balance of office-based, home-working and community-based practice.

The Role

As a Social Worker within the CMHT, you will:

  • Work with adults, carers and families to assess needs, strengths and risks and identify appropriate support.

  • Act as a Care Coordinator, developing and reviewing person-centred care plans.

  • Support individuals to maintain independence and achieve positive outcomes within their communities.

  • Assess and manage risk, including safeguarding and the protection of vulnerable adults.

  • Work closely with health professionals, carers and partner agencies to coordinate effective packages of care.

  • Ensure individuals' views, wishes and rights remain central to planning and decision-making.

  • Maintain accurate case records and complete all statutory social work responsibilities.

  • Work within relevant legislation, including the Social Services and Well-being (Wales) Act 2014 and Mental Health Act 1983.

  • Manage your own caseload with support and supervision from the Team Leader/Senior Practitioner.

Working Arrangements

This is a full-time position for an initial 3 months.

The role offers a flexible hybrid working arrangement, with the expectation that you will:

  • Be office-based in Swansea at least one day per week, covering duty.

  • Spend part of your working week visiting service users in their homes and the community.

  • Work from home for the remainder of your working week, where appropriate.

This is a great opportunity for someone who enjoys community-based social work but also values the flexibility of hybrid working.

To Apply

To be considered for this role, you will need:

  • A minimum of 6 months' experience working in the UK as a registered Social Worker

  • A relevant Social Work qualification

  • Current registration with Social Care Wales or Social Work England

  • An Enhanced DBS, registered on the Update Service, or be willing to apply for one

  • 3 years' referencing history

  • A full driving licence and access to a car
