Longer Term Team - Social Worker

Pay rate - £35 Umbrella / £26.8 PAYE

Vale of Glamorgan Council 

2 days in the office, 3 days from home.

1. To undertake assessment of citizens in accordance with statutory duties under relevant legislation. 
2. To plan and commission services to enable adults with care & support needs and carers with support needs to overcome the barriers to achieving the personal outcomes and monitor and review the care plans through co-production with citizens and carers
3. To work in partnership with citizens with care and support needs, carers with support needs  and adults at risk to provide the best opportunities for people to live fulfilled lives in a way that matters to them.
4. To negotiate and co-ordinate applications for resources and to work in partnership with internal and external partners both statutory and voluntary on behalf of citizens and carers
5. To undertake case management responsibility for a specific caseload and prioritise workload demands appropriately.
6. To maintain up to date electronic - case files and relevant documents, providing accurate records relating to performance management and other statistical data as required.
7. To prepare for and participate in supervision and Personal Development Review (#Its About me) sessions with supervisor/ line manager, and identify personal development and training needs to maintain Registration with Social Care Wales.

• Registration with Social Care Wales
• Enhanced DBS registered on the update service or be happy to apply for a DBS
• 3 Years of referencing
