Hoop Social Work is delighted to be working with Cardiff Council to recruit an experienced Community Mental Health Social Worker to join their well-established Community Mental Health Team.

This is an excellent opportunity for an experienced practitioner who enjoys working in a fast-paced, person-centred environment, supporting adults with complex mental health needs to maximise their independence and achieve positive outcomes.

The Role

As part of the Community Mental Health Team, you will:

• Complete strengths-based assessments and care planning.

• Assess, commission and review care and support packages.

• Work within current legislation, policy and statutory guidance.

• Undertake planned interventions to promote independence and wellbeing.

• Safeguard adults who may be at risk or vulnerable.

• Work collaboratively with health professionals and partner agencies to deliver effective multi-agency support.

• Empower individuals to have choice, control and a strong voice in decisions affecting their lives.

• Contribute to maintaining high-quality services across the city.

To apply, you'll need:

• A recognised Social Work qualification.

• Registration with Social Care Wales or Social Work England.

• At least 6 months' UK experience working as a registered Social Worker.

• An Enhanced DBS on the Update Service (or be willing to obtain one).

• Three years of satisfactory references.

• A full UK driving licence and access to a vehicle.
