£38.43 per hour Umbrella
Full-time | Ongoing contract | 1 day per week on site

Hoop Social Work are recruiting for a Social Worker to join the Adults with Learning Disabilities Team in Rhondda Cynon Taf on an ongoing contract basis.

This role is based within the Complex Learning Disability Team at Ty Elai, Williamstown, Rhondda, covering the West of the county. This is a full-time position and the team work on a hybrid basis, with 1 day per week on site and the remainder worked remotely.

We are looking for an experienced and committed Social Worker to join a specialist and supportive team working with adults with learning disabilities and additional complex needs. This includes individuals with needs relating to ASD, communication difficulties, behaviours that challenge, and physical and mental health complexities.

The Role

As part of this specialist service, you will hold and manage a caseload of long-term and complex cases, working closely with service users, families, health professionals and partner agencies to deliver high-quality, person-centred support.

Duties will include:

  • Undertaking assessments, reviews and care planning
  • Supporting adults with learning disabilities across a variety of settings, including supported living, residential placements and family homes
  • Working in a multi-disciplinary way alongside the Specialist Learning Disability Health Team and other professionals
  • Safeguarding adults at risk and providing a proportionate and timely response where concerns arise
  • Working within relevant legislation including the Social Services and Well-being (Wales) Act 2014 and the Mental Capacity Act
  • Applying a strengths-based and outcome-focused approach to practice
  • Managing risk effectively and promoting positive outcomes for service users

To be considered for this role, you must have:

  • A Social Work qualification
  • Current registration with Social Care Wales
    Please note, candidates registered with Social Work England only cannot be considered for this post.
  • A minimum of 6 months’ UK experience working as a registered Social Worker
  • An Enhanced DBS on the update service, or be willing to obtain one
  • 3 years’ referencing
  • A full UK driving licence and access to a car
