Part-Time Social Worker | Reablement & Hospital Discharge
Vale of Glamorgan Council – Barry Hospital
18.5 hours per week
Contract until March
Circa £34 per hour
We’re working closely with Vale of Glamorgan Council to recruit an experienced Social Worker to join the Vale Community Resource Service (VCRS) - a brilliant, fast-paced reablement service based at Barry Hospital.
This is a part-time opportunity (18.5 hours), ideal for someone who enjoys meaningful, short-term work that genuinely supports people to regain independence and confidence. This role is primarily on site. Access to a car will be required. Pay rate is between £33-£34 Umbrella company. Newly qualified social workers can be considered, dependent upon placement experience.
About the Service
VCRS is a short-term reablement service supporting individuals who are being discharged from hospital, or need urgent support to prevent hospital admission. The focus is on building confidence, restoring skills, and helping people move forward safely and independently wherever possible.
You’ll be part of a strong multidisciplinary team, made up of both health and social care professionals, working collaboratively to achieve the best outcomes.
The Role
As one of four Social Workers within the team, you will:
- Hold a medium caseload of individuals who require longer-term support
- Provide advice and guidance to the wider VCRS team
- Set up long-term care packages, predominantly at home, with some respite or permanent placements where needed
- Ensure a smooth transition from VCRS support workers to private care agencies
- Support individuals to understand their options and shape outcomes that matter to them
- Review care arrangements and continue to assess whether further progress and independence can be achieved
This is a role that blends professional judgement, compassion, and partnership working with real impact for people at a key point in their lives.
To apply for this role, you will need to hold -
- A relevant Social Work Qualification
- Registration with Social Care Wales / Social Work England
- Statutory Adults Services experience in the UK
- Enhanced DBS, registered on the update service or be happy to apply for a DBS
- 3 Years of referencing
- Driving license with access to a car
