Job Description

Supported Care Planning - Social Worker

 

Pay rate in line with the pledge - £38.43 Umbrella / £30.50 PAYE

 

Swansea Council 

 

The current working arrangements are office days Monday's, Wednesday's and Friday'sFriday is a lock down day where the whole team are in the office (Guildhall, Swansea, SA1 4PE). 

 

1. Work with individuals, families, groups and communities to assess their needs and circumstances.

2. Plan, carry out, review and evaluate social work practice with individuals, families, groups, communities and other professionals using the Signs of Safety methodology.

3. Support individuals by respecting their needs, views and circumstances.

4. Assess risk to individuals, families, groups, communities and take appropriate action to minimise such risk in accordance with the Framework for Assessment, conducting Sec 47 Enquiries and undertaking Assessments, including via visits to service user homes, and within the PLO and Court arena.

5. Manage and be accountable (with supervision and support) for your own social work practice within your organisation.

  • A relevant Social Work Qualification
  • Registration with Social Care Wales
  • Enhanced DBS registered on the update service or be happy to apply for a DBS
  • 3 Years of referencing
  • Driving license with access to a car
