This post sits within the Leaving Care Service, where the successful candidate will manage a caseload of young people preparing to leave care. Working alongside dedicated Pathway Advisors who are co-located within the service, the role involves early engagement with young people transferring from other services.

The Social Worker will lead on completing pathway plan assessments and initial pathway plans, maintaining these in line with statutory requirements up to the young person's 18th birthday, at which point full responsibility transfers to their allocated Pathway Advisor.

In addition to caseload management, the post holder will contribute to the team's duty system, providing additional support to young people alongside the Pathway Advisors.

• A relevant Social Work Qualification
• Registration with Social Care Wales
• Enhanced DBS registered on the update service or be happy to apply for a DBS
• 3 Years of referencing
• Driving license with access to a car
