Senior Practitioner - Home to Decide Team

 

Pay rate from £30.88

 

South Gloucestershire Council 

 

Social Work Senior Practitioner required to support the Team Manager and Team members in a team that supports the discharge to access pathway one. 
The Home to decide team is a fast paced team that receive referrals from Sirona care and health following a period of rehab after an acute hospital admission. 
The team is a assessment service that provides support at home whilst the assessment is ongoing. 
As a senior you will be required to support and supervise practitioners in the team, as well as supporting care staff and the business support team. 
You will be the link between the health team and local authority taking part in daily huddles to assist flow through to social care. 
You will need to be able to carry out visits with team members to support them with any complex cases and decision making.
You will need to assist the team manager with data to demonstrate flow and  be proactive in supporting  our health colleagues ensuring that the person receiving the care is getting the right support at the right time.

To apply for this role, you will need to hold -

  • A relevant Social Work Qualification
  • Registration with Social Care Wales
  • Enhanced DBS registered on the update service or be happy to apply for a DBS
  • 3 Years of referencing
  • Driving license with access to a car
