Palliative Care Social Worker
Gwent
Maternity cover contract Jan 2026 – Mar 2027 (funding dependent)
£38,898 – £45,637
30 hours per week

We’re delighted to be partnering with a truly special community-based service supporting people and families across Blaenau Gwent living with life-limiting illnesses.
They’re now looking for a Palliative Care Social Worker to join their warm, multidisciplinary team on a maternity cover contract starting January 2026.

The Role
- You’ll provide psychosocial and bereavement support to Hospice patients, families, carers, and those who are bereaved helping people navigate challenging life circumstances with empathy and skill.
- You’ll Complete holistic assessments and create tailored support plans.
- Deliver evidence-based 1:1 and group interventions.
- Work collaboratively with professionals across health, social care, and community settings.
- Safeguard and advocate for those in your care, ensuring their voices are heard.
- Facilitate group work, reflective sessions, and contribute to service development.
- You’ll hold a manageable caseload of 10–20, so you can focus on meaningful relationship-based practice rather than paperwork.
What We’re Looking For
To apply, you’ll need:
- A relevant Social Work qualification and registration with Social Care Wales
- Experience working in multi-disciplinary and partnership settings
- Strong skills in assessment and intervention, including complex cases and mental capacity assessments
- Knowledge of: the Social Services & Well-being (Wales) Act 2014, Mental Capacity Act and Mental Health Act, Mental Health (Wales) Measure 2010, The welfare benefits system, The United Nations Principles for Older and Disabled People
- Evidence of innovative practice, research or audit experience, and ability to translate evidence into practice
- Excellent communication, presentation, and organisational skills
- IT proficiency and confidence managing your own workload
- Experience of delivering learning opportunities to others
