Mental Health Team - Social Worker

 

Pay rate - £32.65 Umbrella

 

South Gloucestershire Council 


This is a mental health social worker within Adult Social Care

This role will be based at Kingswood Civic Centre, Kingswood, Bristol, BS15 9TR

  • This role is hybrid, predominantly you will be required to work in the office or within the community, with occasional days working at home.
  • Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
  • You will be expected to travel throughout the authority and surrounding area so must have a full valid driving licence with regular access to a vehicle or have an alternative appropriate means of travel. 
Details of the role:

a)To carry out assessments and reviews within the framework of the Care Act 2014 for service users new to the service and to review care packages for those already receiving care packages, to make sure that the agreed plans are implemented.

b)To ensure that care packages continue to meet previously assessed needs and where they no longer do so to reassess and either1) decrease the care package and so avoid disempowerment of service users or 2)increase them with a view to preventing or delaying needs.

c)You will have a working knowledge of the Mental Capacity act and be able where necessary to advise other colleagues in the team in relation to the process of carrying out an assessment for capacity and the process of best interests’ decisions.

d)To operate in line with the council’s safeguarding policy and where necessary to raise safeguarding alerts in relation to service users’ needs for intervention.

e)To carry out carer’s assessments in line with the Care Act 2014 to support them in their caring role.

f)You will carry a caseload which will include more complex cases and be expected to engage with this work from the start of your placement.

g)You will work for the North Recovery team based in Thornbury and your service users will be based in the north area of the local authority

h)To give advice, information and assistance to disabled people and carers so that they are aware of their rights and entitlements, and of the content of the care plan.

  • Registration with Social Care Wales
  • Enhanced DBS registered on the update service or be happy to apply for a DBS
  • 3 Years of referencing
  • Driving license with access to a car
