2 Social Workers are required to join a Hospital Discharge team for an initial 3 month contract with potential to extend. Dependent upon fluidity you could be based at either St Davids Hospital in the City Centre, Llandough Hospital or UHW. This is a fantastic opportunity to join an experienced and supportive team working in a fast paced environment to affect timely discharges from hospital. Pay rate for this position is £35 per hour as an umbrella company worker.

• Ability to work with a wide range of professionals championing the voice of the citizen at all time
• Sound knowledge of the Social Services & Wellbeing Act, Mental Capacity Act, Deprivation of Liberty Safeguards and the Human Rights Act.
• Commitment to working in a strengths based way
• Experience of supervising staff

To apply for this role, you will need to hold -

• A minimum of 6 months experience of working in the UK as a registered social worker
• A relevant Social Work Qualification
• Registration with Social Care Wales / Social Work England
• Enhanced DBS, registered on the update service or be happy to apply for a DBS
• 3 Years of referencing
• Driving license with access to a car
