Hospital Discharge Assessment Team - Social Worker

Pay rate circa - £30 Umbrella

North Somerset Council 

SW required for Hospital Discharge Assessment Team, working with people being discharged from acute hospital beds. In-person assessments in hospital, in the person's home, or in D2A pathway beds in North Somerset and Bristol. Desk work shared between person's home and Weston super Mare Town Hall.

To apply for this role, you will need to hold -

• A relevant Social Work Qualification
• Registration with Social Care Wales
• Enhanced DBS registered on the update service or be happy to apply for a DBS
• 3 Years of referencing
• Driving license with access to a car
