Social Worker – Dementia Support Service Blaenau Gwent | Part-time (22.5 hours)

 Fixed-term until March 2027

£23,558–£28,368 per annum

 

Hoop Social Work are supporting a specialist dementia service in Blaenau Gwent who are looking for a compassionate Social Worker to join their dedicated team. If you’re passionate about person-centred care and enjoy working closely with families and multidisciplinary colleagues, this could be a really rewarding next step. This role is predominantly based on site in the Blaenau Gwent area and requires travelling to clients across the County. This is a part time position for 22.5 hours per week offering a salary of between £23,558 - £28,368 per year.

 

About the role

You’ll be working with people living with dementia, along with their families and carers, offering psychosocial support and helping them navigate challenges with dignity, confidence and independence.

The service is small, warm and highly collaborative, with reflective practice and holistic care central to its approach.

You’ll complete detailed assessments, provide appropriate interventions and support individuals with complex needs, always focusing on what matters most to them.

What you’ll need

• Social Work qualification and registration with Social Care Wales

• Experience in dementia care, palliative care or adult social care is desirable

• Strong assessment, problem-solving and decision-making skills

• Excellent communication and interpersonal skills

• Ability to work both independently and as part of a multidisciplinary team

• Full UK driving licence and access to a car
