Home Findings & Partnerships Officer
Pay rate - £21.04 Umbrella / £16.62 PAYE

For the first few weeks you will be fully office based, but following this you will be able to work from home 2 days a week.

To commission homes that are robustly matched to meet the needs and outcomes for children and young people.
Work with internal and external partners and stakeholders to commission homes based on Indicative Budgets, identifying and brokering providers and negotiate individual placements and packages and ensure compliance with Contract frameworks, including notice period.
What this looks like:
* Receiving referrals for CYP outlining their interests, wishes and challenges. As an officer you will ensure the referral is strength based.
* Engage with potential homes/providers to explore suitability and share options with SW teams
* you will negotiate on packages and costs to ensure VFM
* you will have a curious nature, to understand what a home is offering
* Report writing - to seek approval for chosen home
* Record keeping skills with accuracy including data collection on excel
* You will engage with colleagues from Health and Education - to ensure CYP have all they need going forward.

To apply for this role, you will need to hold -
- A relevant Social Work Qualification
- Registration with Social Care Wales
- Enhanced DBS registered on the update service or be happy to apply for a DBS
- 3 Years of referencing
- Driving license with access to a car
